Medical Paperwork Required for ALL Tymor Park Summer Camps

MEDICAL PAPERWORK REQUIRED FOR ALL TYMOR PARK SUMMER CAMPS

THREE MEDICAL DOCUMENTS are REQUIRED TO BE SUBMITTED TO THE PARK OFFICE PRIOR TO JUNE 13th, 2025:

1) Current physical form (from child's doctors office),
2) Immunization record (from child's doctors office), and
3) Mandatory Physician's Orders Form(a.k.a. "Over The Counter Medication Form" listing any parent-approved medications and topical ointment directions).  Copies of the Mandatory Physician's Orders Form are available online and also in the Park Office.  This form MUST be filled out and signed by the child's doctor.

...are ALL required to participate in any camp program according to the NYS Health Department.

These forms MUST be faxed, e-mailed, or delivered directly to the park office before 2PM on JUNE 13TH, 2025. If your child's medical paperwork expires before camp has begun (one full year) or we do not have one on file BEFORE JUNE 13TH, 2025, they will not be allowed to attend camp. No Refunds, NO EXCEPTIONS.
The Recreation Department cannot guarantee follow up reminders as it is a parent responsibility to make sure all paperwork is received.  Thank you.
